Social Support Program Worker (0.4 FTE)

An exciting opportunity exists for an adaptable and energetic Social Support Program Worker to join our Social Support Program team located in Gisborne.

 

About You:

  • You have energy and experience working in Social Support Programs with a diverse range of participants including CHSP, HCP, HACCPYP.
  • You work effectively with the clients and program volunteers to deliver quality recreational activities which meet the client’s  goals.
  • You can assess and apply care planning.You work positively and collaboratively with team members.
  • You model dignified and respectful interactions with members of the broader community.
  • You demonstrate confidentiality and diversity awareness.

 

Key Responsibilities:

  • Plan and regularly review with customers identified goals and plans which contribute to a life of meaning for that individual and which extends their community integration
  • Provide high quality recreational activities to participants in a group setting
  • Work in a collaborative manner to implement activity programs to meet the needs of participants that facilitates participation and achievement
  • Work within a Wellness and Reablement framework to provide support to participants and family to maintain their independence, including choice and decision making
  • Maintain accurate documentation
  • Provide program planning and implementation support

 

To be successful, you will have:

  • Experience in providing social support within a group setting
  • Demonstrated excellent written and verbal communication skills with the ability to collaboratively in a team
  • Strong computer literacy skills
  • A current Working with Children Check
  • First Aid & CPR certificates.
