We have a rare opportunity for a Casual Intake Officer to join our team, based in Gisborne. Intake is a critical part of the MRH business, as it ensures new and existing clients are able to access the appropriate service in a timely manner.
The ideal candidate will work with our motivated and friendly Intake team to support the increase of referrals. We are seeking a creative, passionate and organised individual that has worked in a fast pace and complex environment with various service providers and funding obligations. You will:
- Deliver client focused intake services and service coordination functions
- Undertake initial needs identification, client eligibility for services, client priority status for appointments and potential need for referrals
- Facilitate access for clients to the services they require in a timely manner
- Provide information to members of the community about MRH & others services
- Providing timely feedback to service providers regarding client status
What we are looking for:
- Experience in a Community Health Setting or similar environment, an understanding of the principles of Health Promotion, Active Service Model, Social Model of Health
- Knowledge and/or experience in Disability services
- An understanding of My Aged Care
- The Ability to liaise across the full spectrum of health and welfare services
- Commonwealth Home Support Program (CHSP) and the National Disability Insurance Scheme (NDIS) experience is highly regarded
